CASE SYNOPSIS
PATIENT HISTORY: Ms. W is a 45-year-old woman with a recent diagnosis of a 0.7cm., grade II, ER/PR negative, Her2/neu negative, infiltrating ductal carcinoma of the right breast, with associated high-grade carcinoma in situ. She is status-post excisional biopsy with clear margins. She was referred for cancer risk assessment by her oncologist due to potential hereditary cancer risk.
FAMILY HISTORY: Ms. W has a 17- year-old daughter and a 12-year-old son who are in good health. She has two sisters, ages 42 and 48, and a 40-year-old brother who are all in good health. She does not communicate with her 42-year-old sister. The patient's mother was diagnosed with breast cancer at age 52 and is currently 71 years of age. There are two maternal aunts, one with no cancer history at age 64, and one age 67 with a history of endometrial cancer at age 49, treated with a Hysterectomy and bi-lateral Salpingo-oophorectomy (Hyst/BSO). The patient's 70-year-old maternal uncle was diagnosed with colorectal cancer at age 56. This uncle has a 49-year-old daughter who died of ovarian cancer at age 46, and two unaffected sons in their mid- 40s. The patient's father is 72 years of age. He was diagnosed with prostate cancer at age 69 and is currently without evidence of disease. His brother died of pancreatic cancer at age 76. He has one 70-year-old sister who was diagnosed with breast cancer at age 50, and another sister who died from complications of childbirth at age 32. To the patient's knowledge there is no other history of cancer in the family.
