Self-Tracking Progress for Quality Practice Certification
CCGCoP Genomic Cancer Risk Assessment (GCRA) Quality Practice Certificate
This Self-Report Tracking Mechanism is designed to help CCGCoP Members track their progress toward completing the 20 components required towards obtaining a the Bi-Annual CCGCoP Genomic Cancer Risk Asessemnt Quality of Practice Certificate.
This Quality of Practice Self Reporting and Tracking is based on the honor and integrity system presumed to be acknowledged by all CCGCoP Members when they first accepted the Terms in Conditions for the CCGCoP Portal.
Upon completion of all components, a Certificate will be available for download and print. We encourage everyone to review their Profile to assure their names and credentials are up-to-date prior to completion of all components.
Successful fulfillment of the CCGCoP activities below within the course of 12 months qualifies you to receive our bi-annual GCRA Quality Practice Certificate, which documents your commitment to ongoing practice-centered learning and excellence in GCRA services at your institution. NOTE: Minor changes may be made in these requirements as we develop this certificate program
1. GCRA Case Conference Sessions
a. Attend at least 5 live Web-based GCRA Case Conference sessions offered at City of Hope, University of Chicago, The US Oncology Network or other CCGCoP Case conference setting (new settings to come by end of 2019)
b. Present at least 2 cases during any of the live GCRA sessions
- NOTE: Cases must be submitted via the Case Submission form at: https://redcap.coh.org/surveys... (Link also located on the CCGCoP Portal)
c. Complete a Case Presentation Follow-up Form for at least one of the cases you present in a CCGCoP Case Conference
2. Post at least 5 times on the CCGCoP Discussion Board
3. Topics in Cancer Genomics Research (TICGR) Webinar
A. Attend at least 2 live TICGR sessions
B. Lead/present at least one TICGR session. NOTE: You may also choose to complete an alternative Gene Review activity to fulfill the presentation requirement.

4. Conduct one or more professional and/or lay educational outreach activity related to hereditary cancer/GCRA
Example: CME Grand Rounds, Tumor Board, professional conference, other professional education venues, community, church groups, other patient or lay public avenues)
5. Complete the following surveys:
a. Professional Practice
b. Professional Self-efficacy Survey (1st year IC alumni only)
